Jesse Wilson,
Nan GreenAs the Administrator for Millcreek Home Health and Hospice, Jesse brings a wealth of knowledge from his experience in home health and hospice. Jesse has a Bachelor’s Degree in Business Administration from the University of Phoenix. He enjoys serving and helping others reach their potential. He began working in the home health and hospice industry approximately nine years ago. Jesse is married to his high school sweetheart, Teresa. They have 3 sons and one daughter in law. He enjoys living an active lifestyle where he can play some basketball and do some fishing with his family.

Danesh Rahimi, M.D.
Danesh RahimiDr. Rahimi is Board Certified in Internal Medicine, Geriatric Medicine and Hospice and Palliative Medicine. His extensive history and education, coupled with his comprehensive understanding of geriatric medicine, make him the ideal candidate to direct medical care for Millcreek Home Health and Hospice. Dr. Rahimi is a strong believer in preventative medicine, palliative care and health education. Dr. Rahimi’s expertise leads Millcreek Home Health and Hospice’s team in specializing in symptom management, end of life care, pain control and treatment in a home environment. Dr. Rahimi graduated from the University of Utah Medical School in 1985, and completed his residency in Internal Medicine and his fellowship in Geriatrics at the University of Connecticut.

Dr. Alexander Del Castillo, M.D.
Dr. Del CastilloDr. Alexander Del Castillo is Board Certified in Internal Medicine and Hospice and Palliative Medicine. Dr. Del Castillo brings a strong history of providing exceptional senior care to Millcreek. He earned his Medical Degree from University of Santo Tomas and completed his internship and residency at Metropolitan Hospital and New York Medical College.  With over 20 years of experience in medicine, Dr. Del Castillo helps Millcreek’s hospice team provide the highest quality care possible to our patients.

Laura Chamberlain, Certified Family Nurse Practitioner (NP-C, MSN)  Advanced practice registered nurse (APRN) Advanced Certified Hospice and Palliative Nurse (ACHPN) 
Laura ChamberlainLaura Chamberlain is a certified Family Nurse Practitioner and Advanced Certified Hospice and Palliative Care Nurse Practitioner. She graduated from University of Alabama in Birmingham with her masters in nursing. She has worked at Omega Interventional Pain Clinic doing pain management. In addition, Laura heads Millcreek Primary and Palliative Care where she does house call visits for older adults. Prior to becoming a nurse practitioner, she graduated from the University of Utah with a bachelors in nursing. As a nurse she worked in the hospital on both cardiac and general medical floors. She also worked in the community doing home health and hospice.

Angel Sullivan, Registered Nurse (RN), Masters of Health Administration (MHA/Gre), Bachelor of Science in Nursing (BSN), Certified Lymphedema Therapist (CLT)
Angel SullivanMs. Sullivan holds certifications in both Lymphedema and Hospice and Palliative Care and holds a Master’s degree in Healthcare Administration with an emphasis in Gerontology. She has worked exclusively in the Home Health and Hospice since 2004. Her clinical expertise is critical in her role as a leader, educator and supervisor for nurses and aides at Millcreek Home Health and Hospice. In addition to being part of Millcreek’s management team, she continues to provide exemplary direct patient care.

Rochelle Alonzo, Masters of Science in Nursing (MSN), Registered Nurse (RN, BSN) 
Andrea BarnardMs. Alonzo works as a part of Millcreek's Patient Care Coordination Team, helping to ensure that each new or returning patient receives timely, high quality care. Rochelle earned her Bachelor’s of Science in Nursing from the University of Texas at Arlington and a Master’s of Science in Nursing Education from the University of Texas at Arlington. Before moving to Utah, Rochelle worked as a hospital nurse on a surgical unit, telemetry unit, and postpartum unit. She has home health experience as a case manager and Assistant Director of Nursing back in Texas and as a case manager and admission nurse for Millcreek Home Health and Hospice.

Peggy Riedesel, Physical Therapist (PT), Certified Wound Care Specialist (CWS)
Peggy RiedeselMs. Riedesel has been a practicing physical therapist for 25 years and has more than 9 years of wound care experience. She earned her certification as a Wound Specialist through the American Academy of Wound Management in 1999. Ms. Riedesel has extensive experience in acute out-patient and private practice settings, working closely with physicians, vascular and plastic surgeons, and other specialists addressing complex wounds. Ms. Riedesel is well respected and her services are in high demand throughout the local health care community. Her expert services allow Millcreek Home Health and Hospice’s patients to receive specialized wound management without the overly stressful effort of leaving home.

Millcreek Home Health and Spiritual Care Coordinator
John Crepeau, MA, Board Certified Clinical Chaplain (BCCC)
John CrepeauMr. Crepeau is a Board Certified Clinical Chaplain and has more than 9 years experience in health care, social services, health education and pastoral care. As Chaplain at Millcreek Home Health and Hospice, John coordinates and provides spiritual care and emotional support to our patients, families and staff. He is also the Coordinator of our Bereavement Program. He received a Master of Arts degree in Interdisciplinary Consciousness Studies from John F. Kennedy University where he studied human development from the perspectives of body, mind and spirit. Mr. Crepeau completed the College of Pastoral Supervision and Psychotherapy Clinical Pastoral Education program at the George E. Walen Veterans Administration Medical Center.

Ryan Engar, Licensed Clinical Social Worker (LCSW)
John CrepeauMr. Engar’s is a Licensed Clinical Social Worker (LCSW) who has dedicated his life to working in the field of aging and helping seniors. Ryan studied at Arizona State University and the University of Utah where he successfully completed his Graduate work in Social Work with a primary focus on gerontology. Upon graduation, Ryan pursued a professional license in social work and is currently a Licensed Clinical Social Worker (LCSW). Ryan is completing his Ph.D. in social work. As Millcreek Home Health and Hospice’s LCSW, Ryan directs the social work department that provides psychosocial support to patients and families, helping them plan and cope with acute, chronic, and/or terminal illness.

Natalie Green, Master’s of Public Administration (MPA)
Natalie GreenMs. Green has worked for Millcreek Home Health and Hospice since 2007 and is currently our Organizational Manager. Ms. Green is an accomplished public speaker who works both in the office and in the community. She plays a key role in the systematic, organization-wide, effort to increase effectiveness and efficiency. She manages the company’s strategic goals while optimizing workplace productivity and management. She holds a Master’s of Public Administration from the University of Utah.
