Primary and Paliative Care


As you age daily activities often become more difficult. The thought of getting ready, driving to the Doctors Office, waiting to be seen and driving home can be overwhelming. The hassle that is often associated with going to see the doctor can result in patients procrastinating and delaying physician visits which may be putting their health at risk.

When medical providers see you in your own home, the stresses related to traditional physician offices are alleviated. Physicians and nurse practitioners who make house calls can see you when functional limitations prevent you from getting out to your doctor. Medical care can be effectively provided by medical professionals in patients’ homes. At home medical providers become familiar with patient’s families, they learn their values and preferences because they are able to see patients in their own environment not in a typical office setting. Millcreek Primary and Palliative Care has a dedicated Nurse Practitioners who make house calls.

Laura Chamberlain is a Certified Family Nurse Practitioner heading our Primary and Palliative care program. Ms. Chamberlain holds advanced certifications as an Advanced Practice Registered Nurse (APRN) Advanced Certified Hospice and Palliative Nurse (ACHPN). She graduated from the University of Utah with a bachelors in nursing and The University of Alabama in Birmingham with her masters in nursing. She has worked at Omega Interventional Pain Clinic where she gained a deeper understanding of pain management. This expertise has become invaluable for all patients as they navigate chronic disease and symptom management. In addition, Ms. Chamberlain makes house call visits for older adults when physical limitations prevent them from getting to their doctor. Laura has worked with Millcreek Home Health and Hospice for over 7 years and has a longstanding presence in the Greater Salt Lake Valley community.

As a Nurse Practitioner Ms. Chamberlain can assume primary care for you and provide this benefit in your home. She can complete a thorough evaluation and assessment, prescribe medication, write orders for home health and hospice, and much more. Ms. Chamberlain also provides palliative care, which is the medical term used to describe the comfort achieved by patients through effective pain control and symptom management. Palliative care is aimed at improving comfort and providing dignity, not reversing the underlying disease. Palliative care is part of the continuum of care that Millcreek provides through guided support and symptom management for individuals suffering from a serious illness. Palliative care is a comprehensive medical approach working with doctors, nurse practitioners, nurses and other medical professionals.
